Objective – To develop a recording card presenting the damages in violence and accident victims in urgency and emergence services and to evaluate the interobservers reproducibility in its use. Methods – The first version of the recording card was built through a bibliographic research about the theme. Afterwards, it was evaluated by specialists and tested in a sample study. Lately it was filled in by patients who suffered injuries by external causes. Seventy-three victims were interviewed and re-interviewed. It was performed the reproducibility measurement through the agreement percentage and the kappaindex adjusted and gross. Results –The recording card was easily fulfilled (average = 6 minutes 54 seconds; median – 5:00 minutes). The crude proportion of agreement record/re-interview ranged between 40 and 100%. According to the Kappa Agreement Index, indicators as aggressions and discharge obtained the worst results (0,37 and 0,34). The others kept the agreement index between 0,68 and 1,00. After adjusting kappa to bias and to prevalence, results of better quality were obtained (between 0,73 and 1,00). Two questions needed to be restructured. Conclusions – The instrument was built and in the most of the questions in-terobserves reproducibility was obtained.
