Objective – To evaluate the work of the Health Strategy of the São Joaquim de Porteirinha family, as to action taken to improve adhesion and control of hypertension among the elderly. Methods– This is a descriptive study of qualitative approach. Data were collected from March to April 2016 with 38 elderly patients with hypertension what underwent a semistructured interview. Results – It was observed that the respondents consider important the assistance, but identified the need to change the times at which the actions/measures are carried out, performing more actions in groups and of streamline such moments. The professionals do not involve relatives at the actions focused for the elderly bearer of hypertension patients, lack basic pharmacy medicines and exams. Said they are carried out medical and nursing consultations, guidelines and measurements of blood pressure, guidance on the use of medications, operative groups, registrations and monitoring of bearer of hypertension patients, dispensing medicines and said what their health needs are met, despite the limitations of the service. Conclusions – The professionals have strategies to promote adherence to treatment and efective control of hypertension. However, it is necessary to reinforce that assistance to bearer of hypertension consists of actions and multi measures that go beyond just informing, should be routinely verified the understanding and treatment adherence by users, as well as make adjustments in the logistics of care.
