The goal of this randomized, blinded, split-mouth controlled clinical trial was to assess the influence of abutment surface treatment on tissue healing. Fifteen patients received two implants distributed randomly to two groups: test (TiO2 abutment surface), control (standard abutment surface). Levels of epidermal growth factor (EGF), bone morphogenetic protein 9 (BMP-9), endothelin 1 (ET-1), fibroblast growth factor (FGF), placental growth factor (PlGF), and vascular endothelial growth factor (VEGF) were quantified in the peri-implant fluid after 3, 14, 30, and 60 days. Inter-group comparisons indicated higher levels of EGF, BMP-9, ET-1, FGF, and PlGF in the test group after 30days (P<0.05). PlGF levels were also higher in the test group after 60 days. In the test group, intra-group analysis revealed different levels of ET-1 and FGF between days 3 and 30, and days 3 and 60 (P<0.05); furthermore, VEGF levels were significantly higher on day 60 than on day 3 (P <0.05). In the control group, intra-group analysis demonstrated significantly different levels of ET-1, FGF, and PlGF between days 3 and 60 and of PlGF between days 14 and 60 (P<0.05). In conclusion, abutment surfaces treated with TiO2 influenced the levels of angiogenesis and bone-related markers.
Resumo
O objetivo deste ensaio clínico randomizado, cego, controlado e do tipo boca dividida (split-mouth) foi avaliar a influência do tratamento da superfície dos pilares protéticos (abutments) sobre o processo de cicatrização dos tecidos peri-implantares. Quinze pacientes receberam dois implantes cada, distribuídos aleatoriamente em dois grupos: grupo teste, com pilares tratados com dióxido de titânio (TiO₂), e grupo controle, com pilares de superfície convencional. Os níveis de fator de crescimento epidérmico (EGF), proteína morfogenética óssea 9 (BMP-9), endotelina-1 (ET-1), fator de crescimento de fibroblastos (FGF), fator de crescimento placentário (PlGF) e fator de crescimento endotelial vascular (VEGF) foram quantificados no fluido peri-implantar aos 3, 14, 30 e 60 dias. As comparações entre os grupos demonstraram níveis significativamente mais elevados de EGF, BMP-9, ET-1, FGF e PlGF no grupo teste após 30 dias (P < 0,05). Além disso, os níveis de PlGF permaneceram significativamente mais elevados no grupo teste após 60 dias. Na análise intragrupo, o grupo teste apresentou diferenças significativas nos níveis de ET-1 e FGF entre os dias 3 e 30, bem como entre os dias 3 e 60 (P < 0,05). Também foi observado aumento significativo dos níveis de VEGF aos 60 dias em comparação aos 3 dias (P < 0,05). No grupo controle, a análise intragrupo revelou diferenças significativas nos níveis de ET-1, FGF e PlGF entre os dias 3 e 60, além de diferenças nos níveis de PlGF entre os dias 14 e 60 (P < 0,05). Concluiu-se que o tratamento da superfície dos pilares com TiO₂ influenciou positivamente os níveis de marcadores relacionados à angiogênese e ao metabolismo ósseo, sugerindo um efeito favorável sobre o processo de cicatrização peri-implantar.
