The aim of this study was to evaluate the effect of photodynamic therapy (PDT) on non-surgical periodontal treatment in patients failing highly active antiretroviral therapy (HAART). comparing methylene blue (MB) (0.01%) and malachite green (MG) (0.01%) dyes as a therapeutic protocol, measuring the following periodontal clinical parameters: clinical probing depth (CPD), gingival recession (GR), clinical attachment level (CAL), full-mouth plaque index (PI), and full-mouth bleeding index (BI), using polymerase chain reaction (PCR) for identification of Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), and Tannerella forsythia (Tf) pathogens. The study was approved by the Research Ethics Committee. Forty-four patients were evaluated, 15 (34.1%) female and 29 (65.9%) male. The mean age was 46 years. In the evaluation of GR, there was a significant difference in the non-resistant group in which MB was used. Resistant patients treated with MG or MB showed significant differences in the decrease of Pg bacteria. PDT associated with MB promoted improvements in CPD and decrease of Aa and Pg bacteria, being an effective therapeutic protocol.
Resumo
O objetivo deste estudo foi avaliar o efeito da terapia fotodinâmica (PDT) no tratamento periodontal não cirúrgico em pacientes com falha na terapia antirretroviral de alta potência (HAART). Comparou-se o uso dos corantes azul de metileno (AM) (0,01%) e verde malaquita (VM) (0,01%) como protocolo terapêutico, mensurando os seguintes parâmetros clínicos periodontais: profundidade de sondagem clínica (PSC), recessão gengival (RG), nível de inserção clínica (NIC), índice de placa (IP) e índice de sangramento (IS), utilizando a reação em cadeia da polimerase (PCR) para identificação dos patógenos Aggregatibacter actinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg) e Tannerella forsythia (Tf). O estudo foi aprovado pelo Comitê de Ética em Pesquisa. Quarenta e quatro pacientes foram avaliados, sendo 15 (34,1%) do sexo feminino e 29 (65,9%) do sexo masculino. A média de idade foi de 46 anos. Na avaliação da GR, houve diferença significativa no grupo não resistente em que o MB foi utilizado. Pacientes resistentes tratados com MG ou MB apresentaram diferenças significativas na redução da bactéria Pg. A PDT associada ao MB promoveu melhorias na CPD e redução das bactérias Aa e Pg, sendo um protocolo terapêutico eficaz.
