Considering the absence of predictable and effective therapeutic interventions for the treatment of peri-implantitis, scientific evidence concerning the host response profile around dental implants could be important for providing in the future a wider preventive and/or therapeutic window for this peri-implant lesion, indicating biomarkers that provide quantifiable measure of response to peri-implant therapy. Moreover, a better knowledge of pattern of host osteo-immunoinflammatory modulation in the presence of peri-implantitis could either benefit the early diagnostic of the disease or to cooperate to prognostic information related to the status of the peri-implant breakdown. Finally, new evidences concerning the host profile of modulators of inflammation and of osseous tissue metabolism around dental implants could explain the individual susceptibility for developing peri-implant lesions, identifying individuals or sites with increased risk for peri-implantitis. The focus of this chapter was, based on a systematically searched and critically reviewed literature, summarizing the existing knowledge in the scientific research concerning the host osteo-immunoinflammatory response to the microbiological challenge related to periimplantitis.
Resumo
Considerando a ausência de intervenções terapêuticas previsíveis e eficazes para o tratamento da peri-implantite, as evidências científicas relacionadas ao perfil da resposta do hospedeiro ao redor de implantes dentários podem ser fundamentais para ampliar, no futuro, as possibilidades de prevenção e tratamento dessa lesão peri-implantar. Além disso, essas evidências podem contribuir para a identificação de biomarcadores capazes de fornecer medidas quantificáveis da resposta à terapia peri-implantar. Um melhor conhecimento do padrão de modulação osteoimunoinflamatória do hospedeiro na presença da peri-implantite também pode favorecer o diagnóstico precoce da doença e fornecer informações prognósticas sobre a progressão da destruição dos tecidos peri-implantares. Adicionalmente, novas evidências sobre o perfil dos moduladores da inflamação e do metabolismo do tecido ósseo ao redor dos implantes podem explicar a suscetibilidade individual ao desenvolvimento de lesões peri-implantares, permitindo a identificação de indivíduos ou sítios com maior risco para o desenvolvimento de peri-implantite. Com base em uma revisão sistemática da literatura, este capítulo teve como objetivo sintetizar o conhecimento científico disponível sobre a resposta osteoimunoinflamatória do hospedeiro frente ao desafio microbiológico associado à peri-implantite, apresentando uma análise crítica das evidências existentes sobre esse tema.
