When a dental procedure, surgical or not, is conducted in the upper jaw, one must consider the anatomical relationship of the maxillary sinus with teeth roots, because the accidental penetration of them or of teeth in the maxillary sinus may occur. This usually results from failure to diagnose and untimely use of surgical maneuvers. If such body is not removed and the communication is not treated, the patient may have complications such as chronic sinusitis. A case of chronic odontogenic sinusitis caused by dislocation of dental fragment into the maxillary sinus diagnosed only after five years of the extraction realization and its treatment is reported.
