Objective – To evaluate the diagnostic performance of the Xpert MTB/RIF Ultra test (Xpert Ultra) for the detection of pulmonary tuber-culosis in PPL. Methods – This is an evaluation of a diagnostic test, carried out from the results obtained in the records of the Myco-bacteria section of the State Public Health Laboratory Dr. Giovanni Cysneiros (LACEN-GO) regarding the execution of Xpert Ultra and solid culture, from November 2019 to April 2020. Results – Sensitivity, specificity, positive predictive value (PPV) and negative pre-dictive value (NPV) and their respective 95% confidence intervals were calculated. In the analyzed period, 324 sputum samples were verified, of which 21 (6.5%) were positive in solid culture and 33 (10.2%) in the Xpert Ultra, whose semiquantitative results were: traces (n= 5), very low (n= 4), low (n= 7), medium (n= 1) and high (n= 16). The diagnostic performance of the molecular test revealed sensitivity of 100% (CI 95% 84-100), specificity of 96% (CI 95% 93-98), PPV 64% (CI 95% 50-75) and NPV of 100% (CI 95-75). 95%100-100). Conclusion – The Xpert Ultra showed high sensitivity and specificity. In additional, the reduction of diagnosis time and ini-tiation of TB treatment. Furthermore, the analysis of the traits category, according to the Ministry of Health algorithm, will avoid possible unnecessary treatments.
