This study, designed as a randomized controlled trial, investigated how the vertical placement of implants influences peri-implant conditions in patients with type 2 diabetes mellitus (T2DM) treated with mandibular overdentures. Twenty-two edentulous individuals with T2DM received two implants each in the anterior mandible following a split-mouth design: one placed at the crestal level (CL) and the other at a supracrestal level (SL). Clinical, immunoenzymatic, and tomographic assessments were conducted at prosthesis placement (baseline) and after 6, 12, and 24 months. Probing depth (PD) and clinical attachment level (CAL) were consistently higher in CL group than SL in all evaluated periods (p<0.05). Immunoenzymatic assay showed that CL implants demonstrated higher concentrations of IL-4, at 6, 12 and 24 months, and IL-21 at 24 months, when compared to SL implants (p0.05). Higher bone loss was detected in the first year of implant loading compared to the second year in both SL and CL group (p<0.05). In the SL group, the mean bone loss increased by 31.22% from baseline to 24 months, corresponding to a change of 0.74 mm. In the CL group, the initial variation was 1.21 mm (69.94%) over the same period. These findings suggest that supracrestal implant placement may offer clinical and immunoinflammatory advantages in patients with T2DM, promoting better peri-implant health and reduced bone loss over time. Within the limitations of this study, SL positioning appears to be a favorable strategy for implant-retained overdentures in diabetic individuals. The clinical trial registry number is NCT03988140. Rehabilitating diabetic patients with dental implants can be challenging. This study demonstrates that supracrestally placed dental implants exhibit better clinical behavior in maintaining peri-implant health.

Este estudo, delineado como um ensaio clínico randomizado e controlado, investigou como o posicionamento vertical dos implantes influencia as condições peri-implantares em pacientes com diabetes mellitus tipo 2 (DM2) reabilitados com overdentures mandibulares. Vinte e dois indivíduos totalmente edêntulos com DM2 receberam dois implantes na região anterior da mandíbula, seguindo um delineamento de boca dividida (split-mouth): um implante foi instalado ao nível da crista óssea (CL – crestal level) e o outro em posição supracrestal (SL – supracrestal level). Avaliações clínicas, imunoenzimáticas e tomográficas foram realizadas no momento da instalação da prótese (baseline) e após 6, 12 e 24 meses. A profundidade de sondagem (PD) e o nível de inserção clínica (CAL) foram consistentemente maiores no grupo CL em comparação ao grupo SL em todos os períodos avaliados (p < 0,05). A análise imunoenzimática demonstrou que os implantes posicionados ao nível da crista óssea apresentaram concentrações significativamente mais elevadas de interleucina-4 (IL-4) aos 6, 12 e 24 meses, e de interleucina-21 (IL-21) aos 24 meses, quando comparados aos implantes posicionados em nível supracrestal (p < 0,05). Em ambos os grupos, a maior perda óssea ocorreu durante o primeiro ano de carga funcional, sendo significativamente superior à observada no segundo ano (p < 0,05). No grupo SL, a perda óssea média aumentou 31,22% entre o baseline e os 24 meses, correspondendo a uma variação de 0,74 mm. No grupo CL, a variação no mesmo período foi de 1,21 mm, equivalente a 69,94%. Esses resultados sugerem que o posicionamento supracrestal dos implantes pode proporcionar vantagens clínicas e imunoinflamatórias em pacientes com diabetes mellitus tipo 2, favorecendo melhor saúde peri-implantar e menor perda óssea ao longo do tempo. Dentro das limitações deste estudo, a instalação dos implantes em posição supracrestal mostrou-se uma estratégia promissora para a retenção de overdentures em indivíduos diabéticos. O estudo foi registrado sob o número NCT03988140. Os achados reforçam que a reabilitação de pacientes diabéticos com implantes dentários pode apresentar melhores resultados clínicos quando os implantes são posicionados em nível supracrestal, contribuindo para a manutenção da saúde dos tecidos peri-implantares.
