The Acquired Immune Deficiency Syndrome (AIDS) is a condition that manifests itself after the infection of the human organism by the Human Immunodeficiency Virus (HIV). In 1996, the Highly Active Antiretroviral Therapy (HAART) was introduced, with the aim of slowing down the immunodeficiency and restoring the immunity of these patients, extending their life expectancy. Consequently, the need for rehabilitating dental treatments arose, aiming to improve oral health, self-esteem and the quality of life of these patients. This current study was designed to assess vertical dimensional changes in the peri-implant bone level around the placement of dental implants in AIDS patients using HAART. For the bone level evaluation, at first cone-beam computed tomography, panoramic radiography and periapical radiographs were used during the periods at baseline, 2, 4 and 6 months after the implant installation. The images were digitized and analyzed on programs Adobe Photoshop CS5 and Digimizer 3.1.1.0. Were installed 13 implants that presented a peri-implant bone loss average of 0.26 mm in the first bimester, 0.13 mm in the second and 0.18 mm in the third, totalizing a peri-implant bone loss average of 0.57 mm in the semester. Despite the several metabolic changes that can affect these patients due to infection, drug therapy, immune response and the absence of an adequate stability quotient and insertion torque, all implants showed osseointegration, as well as the parameters of clinical success after the installation of the implant, and the degree of bone loss in this period is within the expected according to the research. Oral health professionals should be aware of the possible complications that future HIV patients may have due to their systemic and drug-related condition in association with osseointegration.
Resumo
A Síndrome da Imunodeficiência Adquirida (AIDS) é uma condição que se manifesta após a infecção do organismo humano pelo Vírus da Imunodeficiência Humana (HIV). Em 1996, foi introduzida a Terapia Antirretroviral Altamente Ativa (TARV), com o objetivo de retardar a progressão da imunodeficiência e restaurar a imunidade desses pacientes, prolongando sua expectativa de vida. Consequentemente, surgiu a necessidade de tratamentos odontológicos de reabilitação, visando melhorar a saúde bucal, a autoestima e a qualidade de vida desses pacientes. O presente estudo foi delineado para avaliar as alterações dimensionais verticais no nível ósseo peri-implantar ao redor da inserção de implantes dentários em pacientes com AIDS em uso de TARV. Para a avaliação do nível ósseo, foram utilizadas tomografia computadorizada de feixe cônico, radiografias panorâmicas e radiografias periapicais nos períodos basal e 2, 4 e 6 meses após a instalação do implante. As imagens foram digitalizadas e analisadas nos programas Adobe Photoshop CS5 e Digimizer 3.1.1.0. Foram instalados 13 implantes que apresentaram uma perda óssea peri-implantar média de 0,26 mm no primeiro bimestre, 0,13 mm no segundo e 0,18 mm no terceiro, totalizando uma perda óssea peri-implantar média de 0,57 mm no semestre. Apesar das diversas alterações metabólicas que podem afetar esses pacientes devido a infecção, terapia medicamentosa, resposta imune e ausência de um quociente de estabilidade e torque de inserção adequados, todos os implantes apresentaram osseointegração, bem como parâmetros de sucesso clínico após a instalação do implante, e o grau de perda óssea nesse período está dentro do esperado, de acordo com a pesquisa. Os profissionais de saúde bucal devem estar cientes das possíveis complicações que futuros pacientes com HIV podem apresentar devido à sua condição sistêmica e relacionada a medicamentos, em associação com a osseointegração.
