The study was aimed at comparing implants installed with guided and conventional surgery. Twenty-nine total edentulous patients were selected, and maxillary contralateral quadrants were randomly assigned to static computer-aided implant surgery (S-CAIS): flapless computer-guided surgery, and conventional surgery (CS): flap surgery with conventional planning. Tomography scans were performed at baseline and 10 days after the surgery for deviation measurement, and radiography was done at baseline and after 6 and 12 months, for peri-implant bone level (PIBL) analysis. Peri-implant fluid and subgingival biofilm were collected to evaluate bone markers and periodontal pathogens. S-CAIS showed less linear deviation at the apical point and the midpoint and less angular deviation (p < 0.05), with greater depth discrepancy in the positioning of the platform (p < 0.05). Higher values of vertical PIBL were observed for the S-CAIS group at baseline (p < 0.05), while lower values of horizontal PIBL were observed for CS (p < 0.05). Bone markers and Tf presented higher levels in CS (p < 0.05). Flapless S-CAIS allowed smaller linear and angular deviations than the conventional technique. However, PIBL was higher in S-CAIS; the conventional technique led to a greater angiogenic and bone remodeling activity by elevating the angiogenic levels and bone markers. Evaluating the different implant insertion techniques can guide clinical and surgical regarding the accuracy, the release pattern of bone markers, and the peri-implant bone level.
Resumo
O presente estudo teve como objetivo comparar implantes instalados por meio de cirurgia guiada e cirurgia convencional. Vinte e nove pacientes totalmente edêntulos foram selecionados, e os quadrantes contralaterais da maxila foram distribuídos aleatoriamente entre dois grupos: S-CAIS (Static Computer-Aided Implant Surgery), correspondente à cirurgia guiada por computador sem retalho (flapless), e CS (Conventional Surgery), correspondente à cirurgia convencional com abertura de retalho e planejamento convencional. Foram realizadas tomografias computadorizadas no início do estudo (baseline) e 10 dias após a cirurgia para mensuração dos desvios de posicionamento dos implantes. Além disso, radiografias foram obtidas no baseline e após 6 e 12 meses para análise do nível ósseo peri-implantar (PIBL – Peri-Implant Bone Level). Também foram coletados fluido peri-implantar e biofilme subgengival para avaliação de marcadores do metabolismo ósseo e de patógenos periodontais. O grupo S-CAIS apresentou menor desvio linear no ponto apical e no ponto médio dos implantes, bem como menor desvio angular (p < 0,05). Entretanto, apresentou maior discrepância na profundidade de posicionamento da plataforma do implante (p < 0,05). Foram observados valores mais elevados do PIBL vertical no grupo S-CAIS no baseline (p < 0,05), enquanto menores valores do PIBL horizontal foram observados no grupo CS (p < 0,05). Os marcadores ósseos e Tf (Treponema forsythia) apresentaram níveis mais elevados no grupo CS (p < 0,05). A cirurgia guiada por computador sem retalho (S-CAIS) proporcionou menores desvios lineares e angulares em comparação com a técnica convencional. Entretanto, o nível ósseo peri-implantar (PIBL) foi mais elevado no grupo S-CAIS, enquanto a técnica convencional promoveu maior atividade angiogênica e de remodelação óssea, evidenciada pelo aumento dos níveis de fatores angiogênicos e de marcadores ósseos. A avaliação das diferentes técnicas de inserção de implantes pode orientar a prática clínica e cirúrgica quanto à precisão do posicionamento dos implantes, ao padrão de liberação de marcadores do metabolismo ósseo e às alterações no nível ósseo peri-implantar.
