Diabetes Mellitus (DM) is identified as an important condition that leads to implant complications, and the apico-coronal position and the prosthetic abutment seem to interfere with peri-implant tissue behavior This study aimed at assessing the effect on peri-implant parameters after implant loading of two different methodologies in type two diabetics patients (T2DM) who received implant muco-retained overdentures. Twenty-two mandibular edentulous T2DM received two implants: Test group: prosthetic abutment of a diameter smaller than the platform of the implant and a morse taper subcrestal implant level; Control group: prosthetic abutment of diameter identical to the platform and hexagonal external implant placed at crestal bone level. Clinical, inflammatory, and tomographic evaluations were performed at baseline (after prosthesis installation), and twelve and twenty-four months after implant loading. Test implants presented inferior probing depth and clinical attachment level compared to control at baseline and 12 months (p < 0.05). Test implants presented reduced levels of pro-inflammatory cytokines at 24 months compared to the control implants (p < 0.05). The control group had more changes in bone parameters from baseline to the first and second years (p < 0.05). The test group resulted in reduced bone remodeling and better clinical conditions and positively modulated peri-implant immunoinflammatory molecules. Trial identification UTN code: U1111-1239-3638.

O Diabetes Mellitus (DM) é identificado como uma condição importante que leva a complicações relacionadas a implantes, e a posição ápico-coronal e o pilar protético parecem interferir no comportamento do tecido peri-implantar. Este estudo teve como objetivo avaliar o efeito de duas metodologias diferentes sobre os parâmetros peri-implantares após a carga do implante em pacientes com diabetes tipo 2 (DM2) que receberam sobredentaduras muco-retidas sobre implantes. Vinte e dois pacientes edêntulos mandibulares com DM2 receberam dois implantes: Grupo teste: pilar protético com diâmetro menor que a plataforma do implante e encaixe cônico Morse subcrestal; Grupo controle: pilar protético com diâmetro idêntico à plataforma e implante hexagonal externo posicionado ao nível ósseo crestal. Avaliações clínicas, inflamatórias e tomográficas foram realizadas no início do estudo (após a instalação da prótese) e doze e vinte e quatro meses após a carga do implante. Os implantes do grupo teste apresentaram profundidade de sondagem e nível de inserção clínica inferiores em comparação ao grupo controle no início do estudo e aos 12 meses ( p < 0,05). Os implantes de teste apresentaram níveis reduzidos de citocinas pró-inflamatórias aos 24 meses em comparação com os implantes de controle ( p < 0,05). O grupo de controle apresentou mais alterações nos parâmetros ósseos da linha de base até o primeiro e segundo anos ( p < 0,05). O grupo de teste resultou em menor remodelação óssea, melhores condições clínicas e modulação positiva de moléculas imunoinflamatórias peri-implantares.
